Forms to download (copy) to your PC. The forms are available in PDF or MS Excel 95 formats. This form can be completed and printed. To be recognised as valid, it needs to be signed in your own hand and sent by mail, or handed in at your local office of the VZP insurer.
VZP can accept forms sent through the VZP Portal. A scanned form with a signature in an image format cannot be accepted as a valid document.
